Elizabethton High School

In order to participate in athletics at Elizabethton High School, athletes must complete the attached physical packet.

These physicals are valid from April 15th until the last day of school in the following year. A new physical is required for any athletic events, workouts, or practices after the last day of school. 

All included forms must be completed for a physical to be considered complete.
